All applicants should submit the following information to Melody Carswell at cmcars00@uky.edu. Post-baccalaureate (non-degree) applicants should also make sure they have completed the appropriate application steps to obtain post-bac status through the graduate school (see number 8 below).
1. Contact information, including your name, mailing address, email address, and phone number.
2. Graduate program in which you are currently enrolled. If you would like to pursue the certificate as a non-degree, post-baccalaureate student, simply write Apost-bac.@
3. Undergraduate degree(s) and any other graduate degrees or graduate programs in which you have been enrolled.
4. Graduate GPA. If you have not taken any graduate courses, provide your undergraduate GPA.
5. If currently enrolled in a graduate program at UK, provide the name of your advisor.
6. List of completed statistical and methodological courses.
7. Brief (250 words or less) description of your reason for pursuing the certificate.
8. For post-baccalaureate applicants only, have you completed your application for post-bac status through the graduate school? If not, please go to the graduate school section of the UK website for on-line materials.
